Patient-centered medical home
What is a patient-centered medical home? It’s a model of care where you’ll have access to everything you need, no matter your age or specific needs. This team-based model of care is headed by your primary care provider and includes all your healthcare providers—from specialists to nurse care managers.
Comprehensive, continuous care
With a patient-centered medical home model (PCMH), you are at the center of your healthcare team and all team members are dedicated to keeping you healthy. You will receive comprehensive, continuous care whether you need preventive care, wellness care, acute care, chronic care, behavioral and mental healthcare or end-of-life care. And no matter how many health care providers from our medical neighborhood make up your individual team, your care will be carefully coordinated.
Access to health information
All team members, including you, have access to your electronic health record through MyChart, so everyone has the most up-to-date information and is on the same page. You and your team will have everything necessary to make the best choices regarding your healthcare decisions.
You are a responsible participant
Your medical home is all about you! Everything revolves around your needs and preferences and you, your family and caregivers are fully engaged in the whole process. You are a responsible participant in your own healthcare. What this means to you is a great patient experience, every step of the way!
